What type of information we have

Personal identification information (name, email address, phone number and DOB) is information that, either alone or in combination with other information collected, identifies an individual. Depending on the nature of services we will collect other information necessary to provide these services to you. Sports massage and Pilates consultations will include information such as your medical history. We do not collect any more Personal Data from you than what we have determined is needed for us to provide the Services or that you have decided to share with us to personalise the Services, and to comply with applicable laws.

We receive email questions from current and potential customers. We answer the emails but do not use the email addresses for any other purpose.

How we get the information

We collect data through consent forms for treatment and participation in classes. We collect patient notes on medical health with regards to treatment. These are stored online in a secure cloud storage system

What we do with the information

Kate Disley collects information you provide to us. Typically this is collected via a web application/email or consent form and stored in a secure cloud environment. We will keep your data provided to us for a minimum of seven years (in line with guidance from the SMA - The Association for Soft Tissue Therapists).

Kate Disley uses the information we collect to administer and operate the services provided to you. We do not share your publicly identifiable information with third parties expect with your express permission.

You may request deletion of your personal identifiable information by contacting the data controller at sportsmassagenortheast.co.uk. However any medical records are stored for a minimum of seven years. Once this period has expired, we will delete your data.
